Rhenium-188 HEDP
Indication:
Palliation of painful bone metastases across multiple cancer types (e.g., prostate, breast).
Mechanism:
A beta-emitting diphosphonate compound that localizes in bone lesions, delivering targeted cytotoxic radiation.
Efficacy:
- In a cohort of 27 prostate cancer patients, 76% reported ≥25% pain relief by 12 weeks; 20% became pain-free (Karnofsky score from 74% to 85%) (Kratochwil et al., 2003).
- A larger study (n = 61) across various cancers showed 80% overall pain relief with no significant hematologic toxicity (Palmedo et al., 2001).
Safety:
Mild transient bone-marrow suppression; acceptable safety profile (Kratochwil et al., 2003).
Rhenium-188 Colloid
Indication:
Radiosynovectomy for joint inflammation in haemophilia and rheumatoid arthritis.
Mechanism:
Intra-articular colloid infusion delivers beta radiation to synovial tissue, reducing inflammation.
Efficacy:
- In hemophilia patients, significant reductions in pain, bleeding, and synovial thickness at 12 months.
- 90% colloid retention in joint at 48 hours, with minimal leakage (Laupland et al., 2023; Santoro et al., 2022).
Rhenium-188 Lipiodol
Indication:
Intra-arterial therapy for unresectable hepatocellular carcinoma (HCC), including portal vein thrombosis (PVT).
Mechanism:
Lipiodol carries Rhenium to tumours, enabling localized beta-radiation.
Efficacy:
- A multicentre IAEA Phase II study (n = 185) showed 22–25% objective response (CR + PR), with 1- and 2-year survival of 46% and 23% respectively (Chiesa et al., 2013; Garin et al., 2013).
- A Phase I Lip-Re-01 trial of 188Re-SSS lipiodol demonstrated high in vivo stability (<5% urinary excretion at 48 h) and favorable safety (Garin et al., 2024).
- Studies validate robust beta emission (max ~2.1 MeV; 10 mm range), efficient imaging (155 keV γ), and on-site generator feasibility (Frontiers, 2023; Open Medscience, 2023).
